Birth time must be exact - You will be able to find your birth time on your birth certificate, or can ask a family member is accuracy can be assured.

If you cannot obtain your exact birth time, please note and we will make a best estimate.

PLEASE NOTE: This is a ZOOM Session, I understand that this session takes place fully online via Zoom.

Setup in a comfortable place where you won't be disturbed, and where you can lay down or sit comfortably to receive during the session

I consent to treatment for myself (or minor) and understand that Reiki is a tool and therapy to support my personal development, but is not intended to diagnose, treat, cure, or otherwise supplement any medical or mental health treatment.

I understand that these services are not a substitute for any medical or psychological care. I understand that Reiki can complement any medical/psychological care I am receiving

I understand that participation is voluntary and that at all times I may choose to end my participation. I understand that I may experience 'healing reactions' following the services provided. I understand healing is self-directed process, and each session is unique.

I acknowledge that long-term imbalances in the body sometimes require multiple sessions in order to facilitate the level of relaxation needed by the body to heal itself.

I understand that by providing this informed consent I am assuming full responsibility for my services and I hold harmless both the practitioner Seasons of Being, any affiliate practitioners, and the facility/location where the services are provided.

I understand that any information exchanged during any session or service is educational in nature and is to be used at my own discretion. I am responsible for my own progress, growth, healing, and well-being.

Privacy Notice Acknowledgement: No information about any client will be discussed or shared with anyone or other entities. However, by checking this box I agree to full disclosure between Seasons of Being and the referring practitioner to ensure continuity of care

Cancellation & Refund Policy:

Payment is due at time of service. Session payments are non-refundable.

If you need to reschedule your appointment, contact Amanda at least 48 hours prior to your scheduled session. Day-of sessions may only be rescheduled in the event of an emergency.

We do not offer a cancellation refund - any no calls/no shows will forfeit their session payment in full.
